I've seen hydraulic cylinders cause failures. I've seen them over-extend and cram the throwout bearing into the fingers. I've seen them not mounted correctly also.

Most failures like yours are from the pads coming loose from the clutch disk.
proably a few more than me like you would ever know how many i have done. not that i care if you have done more than i have.
some clutches cannot be installed backwards such as in a jetta because the bolts wouldn't be even close to long enough to try to cram it in backwards. The clutch even tells you to put it in backwards... way to go sachs....
when did i say there couldn't be failures as you stated. i understand soem people don't have as much experience as others and as a result make mistakes, but the list of things you said earlier were not really possible. You said linkage and thats usually known as shifter linking, not clutch. alignment etc.... the stuff you just mentioned, fine obviously they are possible, but what does that have to do with what was said. the issue was the cluch only. I don't know what you are getting at, just seems like you are trying to spitt out a bunch of info to let everyone know how knowledgable you are.
